DMHOA MEMBERSHIP FORM FOR COMMUNITIES

1. Right-click on "Download" link above and save the file to your computer.
2. Double-click on the PDF file to open up Adobe Acrobat Reader.* 
3. In Acrobat, click on the "Fill & Sign" option (see image at right) which is located either near the top menu or at the right of your screen. NOTE: If you are using the latest version of Acrobat, it is in the menu on the left side of the page.
4. Click on "Name" in the form to begin filling in info.
5. Click on each line to fill in your information. You can also click on a small square box where indicated to insert a checkmark. You can also make the type sizes larger or smaller by clicking on the small/large capital A above where you are typing.
6. Under "File", select "Save As" and save to your computer. Use your first and last name as the file name and save.
7. You have the option to email us the filled-in form or print it out and mail it with your check to our office. You can pay using your credit or debit card through PayPal using the link on our Membership page.
